Reduced APR and Fixed Monthly Payments Required Until Paid In Full*
**On qualifying purchases made with your CareCredit credit card account at enrolled provider locations:
• Purchases of $1,000 or more are eligible for a:
- 24 months offer with a 17.90% APR
- 36 months offer with an 18.90% APR
- 48 months offer with a 19.90% APR
• Purchases of $2,500 or more are eligible for a 60 months offer with a 20.90% APR
**Interest will be charged on promo purchases at a reduced APR from the purchase date. Fixed monthly payments are required until paid in full and will be calculated as follows: on 24-month promotions – 4.9876% of initial promo purchase amount; on 36-month promotions – 3.6605% of initial promo purchase amount; on 48-month promotions – 3.0377% of initial promo purchase amount or on 60-month promotion – 2.6997% of initial promo purchase amount. When patients come for their complimentary consultation any and all questions in regards to pricing is addressed at that time.
DEPOSIT AND WHEN PAYMENT IS DUE: To schedule and hold a cosmetic surgery or procedure, a NON-REFUNDABLE deposit is required. This amount is deducted from the total surgery fee. Payment IN FULL for your procedure is due on or before your pre-operative appointment in order to solidify your surgery date.
PAYMENT OPTIONS: Personal check, cashier’s check, or cash.
**Effective 1/1/2023: When paying, the amount is due 21 business days prior to the surgery date. Credit options include Debit, Visa, MasterCard or American Express. If you are paying with a credit or debit card some banks require you to call to increase your daily limit. Please do this before coming to your preoperative appointment.
**Effective 9/1/2021: All cosmetic surgery pricing will be CASH ONLY Pricing. Should you choose to pay with any other form of payment, you will not receive your 4% cash only discount.
We understand that something might arise that may force you to reschedule, postpone or cancel your surgery or procedure. Please understand that these changes affect not only your surgeon, but other patients as well. We appreciate your courtesy.
