FTM Top Surgery (Mastectomy) at Acadia Women's Health

We understand the journey of a transgender patient can be a long and thought-provoking process in order to achieve a body and look which aligns with that person's identity. FTM Top Surgery can assist with this in order to attain a more masculine chest for a transgender male.

Each surgery is individualized, and the surgery technique is based on the patient's needs. Multiple factors affect if an FTM Top Surgery should be performed, including, but not limited to, the duration of a patient's hormone treatment. Generally, the longer duration of time for hormone treatment, the higher a patient's chances are of qualifying for FTM Top Surgery. Ultimately, it is up to the discretion of the doctor to take further steps toward having the procedure.

Top Surgery Techniques

Periareola Incision (Keyhole) Procedure

An incision is made all around the areola. Next, a second larger, circular incision is made and the resulting skin is removed. Liposuction and breast tissue removal may also be performed. The skin is then brought together and connected to the areola. The areola may be decreased in size and made more into an oval shape to match the conformity of the new chest. A Periareola Incision, or Keyhole, Procedure is ideal for men with small or B-sized chests and have good skin elasticity.

Double Incision
Procedure

Two horizontal incisions are made at the top and bottom of the chest to open the skin. Breast tissue is removed after pulling back the skin and liposuction may be performed to adjust the new shape of the chest. Only the skin is affected in the incision during this procedure, not the muscle. A Double Incision Procedure is aimed at men who have medium to large chests.

Buttonhole
Procedure

Two horizontal incisions are made during this procedure similar to the Double Incision procedure. In a Buttonhole Procedure, the incisions are made in the pectoral muscle. Excess skin and breast tissue are then removed, and the nipple/areola is reattached to the nerves and blood supply in order to preserve sensation in the chest.

Inverted T
Procedure

In an Inverted T, or Anchor Scar, Procedure, two horizontal incisions are made during this procedure similar to the Double Incision procedure, and the muscle is not affected. Breast tissue is then removed after pulling back the skin. Inverted T Procedures are perfect for men with medium to large chests and have excess skin and breast tissue.
